So why did my foam pad just start shredding?

I went to practice more with my rotary on a friend's old car, and as I start using a yellow 6.5 lake country pad, it begins to shred as I start polishing, especially when it rubbed against tape. Same thing proceeded to happen to my orange pad.

So I started wondering as to what could be the culprit. I use Meg's APC 10:1 to clean them along iwth a wash with woolite. But I think it's because I used 50:50 isoproyl alcohol/water to wipe down polish last time, and then used the pads again on the same paint. Any input?

I'm positive I've used sufficient polish, and I cleaned it pretty well from the previous detail, so I'm confident that neither of those explanations apply.... Thanks for the input though. I also used a polishing pad, which had more polish baked in from previous details, and it did not shred- it was also not used the last time I used the IPA/water mix.

If you hit anything innapropriate, i.e. mouldings, emblems, bumper corners, something is going to give, especially at a high speed. And more so when there is little lubrication....Not saying this IS what happened, but it may have been damaged from an earlier, unknown mishap?
