Buffer burn?

I'm pretty sure this pic (the white spot is primer) is a good illustration of paint burn, but I'm curious what you more experienced folks think caused this:

That looks like a burn, and the surrounding area to the lower left looks very thin.

How do you do this?

Here are a few common ways to do it:

-Heat from the pad edge can cut through your clear.
-An abrasive pad/product combo.
-Running a rotary too fast (IMO, anything over 1500 is too fast, and there are very few exceptions to this).
-Wet/dry sanding to far.
-Tilting your pad.
-Over polishing your vehicle
 
I agree, the lower left region looks quite thin as well. Please do tell the story behind it!

Looked like wet sanding gone bad to me.
I don't buy the "I only hand washed it" explaination.
 
It looks like he or someoneelse may have been distracted and left the buffer sit a little to long,or placed on the car @ 3000 rpms ;)
