Since the rust is happening to the metal, it's actually rusting UNDER your primer. You just see whats erupting up through the primer and the paint.
You can at least retard the progress of the rust by sanding/grinding it down and applying a rust convertor. But in most cases, the best course of action is to take it to a paint/body shop. Fixing surface rust is a lot harder than most people expect.
