For those whorun a detail business... What kind of forms do you have your customer fill out and reciept?

I dont have my customer fill out anything. Unless they fill in a form on the website to book an appt. I wouldn't want to make them do anything special or different just to have their car detailed. My staff fills out a check-in sheet, and they sign that, and they also sign the reciept prior to the detail to authorize the total charges.

We fill out a work order. It is important to record the clientâ€â„¢s name, address, contact info, work to be performed and a place for them to sign authorizing the work.
