anyone know of any condition/job detail forms?

anyone have any downloadable forms for job description or vehicle condition forms? for example a form i can give to a customer to show them exacly what i am doing to their vehicle? or maybe a vehicle condition form so i can evaluate the auto and mark the defects? i think this would be a great thing to have to us detailers!
 
I started working on so,ething like this a while back but basically just left it as it was. It was on excel and listed by category (Wheels/Tires, Interior, Exterior). A checkbox was designated for areas I would working on and condition of each. Then a 'Comments' area at the bottom of each section would list what I had done.



example-If someone is paying for middle dollar detail, then I would check off those boxes and list the condition of everything then describe in the comments box a high level step by step. A little section at the top would list the make/model/year/color of the auto and date stamp somewhere.



I think that's why I quit working on it. I got stuck in the details.
